• I understand the Registration Fee due at registration is non-refundable and that I am fully responsible for my monthly tuition.
• I acknowledge the risks and hazards of physical injury inherent in training and performance and hereby assume all such risks and hazards; and I hereby release, waive, and agree not to assert against Alamance Fine Arts Academy, its directors, instructors, officers, agents, or employees any claim for injury to said child in consequence of or incident to such training, performance, or other AFAA activities.
• I have read AFAA’s Code of Conduct and agree to abide by the expectations therein.
• Alamance Fine Arts Academy may use my student’s picture in advertisements, literature, or in web content such as on AFAA's website or AFAA's social media posts, as well as events sponsored and conducted by AFAA. (Please let the office know if you do not agree to this)
